Eligibility Requirements
Patients must...
- live in Livingston County,
- have no dental insurance, and
- have an income at or below the following limits:
Family Size Maximum Income 1 $20,400 2 $27,400 3 $34,300 4 $41,300 5 $48,300 6 $55,200 7 $62,200 8+ $69,000
Registration
Patients must...
- register in person before being given an appointment,
- bring proof of residence in Livingston County, such as a utility bill, and
- bring proof of family income (such as last year’s tax return, last three pay stubs, unemployment statement, or social security statement).
Requirements
- Patients will be required to make a co-payment of $15.00 for each visit, which must be prepaid.
- Bring proof of family income and residence to every appointment.
- Patients must arrive 15 minutes before every appointment to fill out paperwork.
- If patient misses an appointment, the co-payment will not be reimbursed and patient will be put at the end of the waiting list.
- Children under 18 must be accompanied by a parent or legal guardian at every appointment.
- Children must be supervised in the waiting area.

If you are eligible for services, please complete a patient information form and a registration form and bring them with you in person to the center. Applications are being accepted as of September 2, 2008.
Please note that services are provided on a limited basis and alternatives may exist that may not be provided within the scope of this center.
